For a booking form:
  • Booking forms are delivered to local schools 3-4 weeks before the holidays - so check your child's bookbag.
  • Join the mailing list for £4.00 per year and forms can be posted or emailed directly to you before they go out to schools and before it is available from the website.  
  • Download the Latest Booking Form by hitting the button below.
  • Phone for a booking form.
  • Pop in to your local club site.
To book:
  • Return your booking form, along with payment to Elm Grove Childcare Club, Elm Grove Primary School, Elm Grove, Worthing, West Sussex BN11 5LQ
  • Return your booking form, along with payment to your local Childcare Club.
